Feb 13, 2024 · Hyponatremia in the syndrome of inappropriate antidiuretic hormone secretion (SIADH) results from ADH-induced retention of ingested or infused water. Although water excretion is impaired, sodium handling is intact since there is no abnormality in volume-regulating mechanisms such as the renin-angiotensin-aldosterone system or atrial natriuretic ...

SIADH (Syndrome of Inappropriate Antidiuretic Hormone Secretion)
https://my.clevelandclinic.org/health/diseases/23976-siadh-syndrome-of-inappropriate-antidiuretic-hormone-secretion
A low blood sodium level (hyponatremia) is the most common cause of symptoms of SIADH. Often, mildly low sodium levels don’t cause symptoms, but moderate to severe hyponatremia can cause the following symptoms: Muscle cramps or weakness. Nausea and vomiting.

Jul 29, 2021 · The SIADH should be suspected in any patient with hyponatremia, hypoosmolality, and a urine osmolality above 100 mosmol/kg. In SIADH, the urine sodium concentration is usually above 40 mEq/L, the serum potassium concentration is normal, there is no acid-base disturbance, and the serum uric acid concentration is frequently low [ 1 ].

Diagnosis is by measurement of serum and urine osmolality and electrolytes. Treatment is with water restriction, sometimes with oral or intravenous sodium chloride, and rarely with vasopressin receptor antagonist medications such as conivaptan or tolvaptan. SIADH can cause significant hyponatremia .
